Glans Filler: Honest Guide to HA Glans Augmentation

Glans filler (glans penis augmentation) is an in-office injection of hyaluronic acid (HA) gel into the head of the penis to make it look fuller and, in some studies, to lower its sensitivity in men with premature ejaculation. It is an off-label use of a temporary, dissolvable filler, and the evidence comes from small clinical series rather than large long-term trials. Results fade gradually, and HA can be broken down with the enzyme hyaluronidase if a problem occurs.

Key takeaways

  • Glans filler uses hyaluronic acid gel, usually around 2 mL, injected into the deep layer of the glans under numbing cream or local anesthetic.
  • Penile use is off-label: the FDA approves dermal fillers for areas such as the lips, cheeks, chin and back of the hand, not the penis.
  • For premature ejaculation, small studies report longer ejaculation times, but the EAU gives only a weak “use with caution” recommendation and the ISSM does not recommend it.
  • The gel slowly absorbs; one 5-year follow-up of 38 men measured about a 15% loss of glans circumference compared with 6 months.
  • Risks include swelling, bruising, uneven contour, nodules, infection, changes in sensation and, rarely, blocked blood flow; HA can be dissolved with hyaluronidase.

What is glans filler?

The glans is the rounded head of the penis. Some men feel it looks small compared with the shaft, or that it does not fill out well during an erection. Also called glans augmentation or penile head enlargement, the procedure places a soft gel under the surface of the glans to add volume. It was developed mainly by a urology group at Korea University.

The gel used in published studies is hyaluronic acid, a substance that occurs naturally in skin. The body gradually breaks HA fillers down, which makes them temporary, unlike silicone or other permanent materials.

An off-label use of an approved product

The US Food and Drug Administration lists approved filler areas such as the lips, cheeks, chin and back of the hand; the penis is not among them. The FDA also states that injectable silicone is not approved for any cosmetic filling. Glans use is therefore off-label, and your doctor should explain this openly.

The two indications discussed in research

1. Cosmetic glans size. In a report of 100 men treated for a small glans, the net gain in maximal glans circumference was about 14.9 mm at one year, and roughly 77% were satisfied.

2. Premature ejaculation (experimental). The idea is that a layer of gel acts as a barrier between touch and the nerve receptors of the glans, reducing sensitivity. Five small studies identified in a 2020 review reported 2.4- to 4.5-fold increases in intravaginal ejaculatory latency time (IELT). However, the review authors judged the method experimental and called for larger randomized trials.

Who is a good candidate for glans penis enlargement

A reasonable candidate is a healthy adult man with a specific concern about glans size or fullness who understands that results are modest and temporary, and accepts an off-label procedure with a limited evidence base.

For premature ejaculation, selection matters even more. Published PE trials enrolled men with lifelong premature ejaculation, often circumcised, without erectile dysfunction or untreated prostatitis. Established options should be considered first; see our premature ejaculation treatment page.

Glans filler vs other options compared

The table places glans augmentation next to related options. Shaft girth is covered on our penile filler (girth enhancement) page and the broader penis enlargement overview.

Option Main goal Evidence and guideline status Reversible?
HA glans filler Fuller glans; experimental for PE Small series and a few trials; EAU weak “use with caution” for PE; ISSM does not recommend Yes, dissolvable with hyaluronidase
HA shaft (girth) filler Increase shaft circumference Randomized trials of short duration; off-label Yes, dissolvable with hyaluronidase
SSRIs and on-demand dapoxetine Delay ejaculation Established medical treatments for PE Effect stops when the drug stops
Topical anesthetic creams or sprays Reduce glans sensitivity before sex Established; can transfer to a partner Yes, short-acting
Behavioral techniques and counselling Improve control (squeeze, stop-start) Established; needs practice Not applicable
Selective dorsal neurectomy Cut sensory nerve branches for PE EAU: do not perform; more safety data needed No

What the guidelines actually say

The European Association of Urology (EAU) notes that randomized studies found HA glans injections reduced penile sensitivity and modestly but significantly raised IELT. It still warns that the procedure may cause serious complications and that more safety studies are needed. Its recommendation is weak: use HA injection with caution compared with more established treatments. The ISSM, as summarized in published reviews, does not recommend it for PE because sensory loss may be irreversible.

The glans filler procedure step by step

Glans filler procedure steps infographic: assessment, numbing, injection, follow-up
The four main steps of a hyaluronic acid glans filler procedure.

The procedure is done as an outpatient. Techniques differ, but published series describe a similar sequence.

  1. Assessment and consent. The doctor reviews your history, medications and expectations, examines the penis, and explains that the use is off-label and temporary.
  2. Numbing. Most patients tolerate a topical anesthetic cream such as EMLA applied for about 30 minutes; some need an injected local anesthetic.
  3. Injection. The gel is placed in the deep dermis of the glans with a fine needle (27G to 30G in published reports). The usual starting volume is about 2 mL. Multiple small punctures, such as 0.25 mL per site, spread the gel more evenly with less pain than long threading passes, which risk tearing the delicate surface.
  4. Shaping. The doctor checks contour and symmetry. Some protocols add a small touch-up about two weeks later. Gel placed too superficially or in excess can cause pressure necrosis, so depth and volume matter.

Recovery timeline and aftercare

Most men return to daily activities quickly. Glans studies rarely spell out aftercare rules, so the abstinence period below comes from penile HA filler studies. Keep the area clean and report fever, spreading redness or color change promptly.

Time after injection What to expect
Same day Numbness wears off; mild soreness, swelling and some discoloration are common.
First 1–2 weeks Swelling and discoloration usually settle within about two weeks; minor bruising or small blisters reported in one PE trial cleared in 7–10 days.
Around 2 weeks Follow-up check; a small touch-up may be offered to smooth uneven areas.
4 weeks Penile HA filler studies asked patients to avoid intercourse for the first 4 weeks; follow your own doctor’s instructions.
6–12 months Main assessment of size and, for PE, ejaculation time.
Years Gradual absorption; re-treatment may be needed to maintain volume.

Risks and side effects of glans filler

Main risks of glans filler and how they are managed.

Published glans series report few serious problems, but they are small, so rare complications may not show up.

Hyaluronidase: the “undo” option

HA can be dissolved by the enzyme hyaluronidase. Glans augmentation authors describe using it for lumpiness or for early discoloration that does not improve with massage and warmth, reporting that most lumpiness resolved within 24 hours. In a shaft series, nodules were treated with small hyaluronidase injections that worked within about three days, sometimes needing one or two repeat sessions. Dissolving the gel also removes the size gain.

Results, durability and realistic expectations

Size

In the original PE study of 139 men, 65 received 2 mL of HA into the glans alone. Their maximal glans girth rose from about 8.95 cm to 11.67 cm at six months. In a separate cosmetic report of 100 men, the average net gain was about 14.9 mm at one year.

Ejaculation time

In that 2004 study, average ejaculation time in the glans-only group increased from about 97 seconds to 282 seconds at six months, with 75% of patients and 62% of partners satisfied. An Italian series of 110 men reported IELT rising from about 88 to 293 seconds at six months, with no adverse reactions, but it had no control group. None of these studies is large or blinded enough to make this a standard treatment.

How long does glans filler last?

The longest follow-up is Kwak and colleagues’ 5-year study of 38 men. Compared with six months, measured glans circumference fell by about 15%, although the men’s own visual rating of the result did not change significantly. Ejaculation latency and vibration threshold declined from their 6-month levels but stayed above pre-treatment values, and 76% of patients and 63% of partners were still satisfied, with no serious adverse reactions. Top-ups may be needed. Facial HA fillers usually last six months to a year, so durability varies.

Limits of the evidence

A 2020 review found only five PE studies, with small samples, varied criteria and non-validated satisfaction tools. In short, the procedure can make the glans fuller for a time and may lengthen ejaculation time in selected men, but it is not a cure.

FAQs about glans filler

Is glans filler safe?

Studies report mostly mild, short-lived side effects and no serious reactions at five years. However, the series are small, the use is off-label, and blocked blood flow is possible with any filler, so the injector’s skill and correct depth matter greatly.

Source: PubMed — Kwak 2008, 5-year follow-up

How much bigger does the glans get?

Gains in published studies were roughly 1.5 to 2.7 cm in glans circumference. Your result depends on anatomy, the volume your doctor considers safe and how quickly your body absorbs the gel, so a personal examination is the only way to estimate it.

Sources: PubMed — Kim 2004, glans HA for PE; PMC — Littara 2013, HA for PE

Does glans filler cure premature ejaculation?

No. Small studies show longer ejaculation times, but the EAU advises using it only with caution and the ISSM does not recommend it. Medication, topical anesthetics and behavioral therapy remain the standard approaches for premature ejaculation.

Source: EAU — Disorders of Ejaculation

How long does glans augmentation last?

HA is gradually absorbed. In the longest study, men kept most of their result at five years, and authors note that timed re-supplementation may be needed to maintain volume. Absorption rates vary between individuals.

Source: PMC — Glans augmentation with HA gel (Moon 2015)

Is the procedure painful?

Most men tolerate it with numbing cream; one trial used 2% lidocaine jelly for 30 minutes. In that study, 10% reported injection-site discomfort, and all minor effects resolved within 7 to 10 days.

Source: PMC — Sakr 2022, glans HA pilot

When can I have sex after the injection?

There is no single standard for the glans. Published penile HA filler studies asked men to abstain from intercourse for four weeks after injection, Always follow the specific timeframe your own doctor gives you.

Source: PMC — HA girth RCT (Ahn 2021)

Can glans filler be reversed?

Yes. HA can be dissolved with hyaluronidase injections, used for lumps, unevenness or compromised blood flow; this also removes added volume. In one penile series, about 30 units were typically used and took effect in around three days.

Source: PMC — Penile HA complications (Quan 2021)

Will it reduce sensation?

Reducing sensitivity is the intended effect when it is used for premature ejaculation, measured in studies as a higher vibration threshold. For men seeking only a cosmetic change, any loss of sensation is an unwanted effect. The ISSM’s concern is that sensory loss may not be fully reversible, so discuss this carefully before treatment.
