Tadalafil Daily Dose: 2.5 mg vs 5 mg, Uses and Safety

Tadalafil daily dose: once-daily 2.5 mg and 5 mg Cialis for erectile dysfunction and BPH
Once-daily tadalafil (2.5 mg or 5 mg) treats erectile dysfunction, enlarged-prostate urinary symptoms, or both. Here is how it compares with on-demand Cialis, what the FDA label and EAU guidelines say, side effects, interactions and who it suits.

Tadalafil daily dose means taking a low dose of tadalafil (Cialis), usually 5 mg and sometimes 2.5 mg, at about the same time every day instead of a larger tablet before sex. The FDA approves taking tadalafil daily for erectile dysfunction (ED), for the urinary symptoms of an enlarged prostate (benign prostatic hyperplasia, BPH), and for men who have both. Because the drug stays at a steady level in the blood, erections can happen whenever you are aroused, without planning around a pill.

Key takeaways

  • For ED, the FDA label starts tadalafil daily at 2.5 mg and allows an increase to 5 mg; for BPH, or ED plus BPH, the dose is 5 mg a day.
  • Tadalafil has a half-life of about 17.5 hours, and daily dosing reaches a steady blood level within 5 days.
  • The EAU finds no clinically significant difference in effectiveness between tadalafil daily and on demand; the main gain is spontaneity.
  • Side effects at daily doses are mostly mild: indigestion, headache, back pain, muscle aches, stuffy nose and flushing.
  • Never combine tadalafil with nitrates, “poppers” or riociguat; daily use means the drug is always in your system.

What is the tadalafil daily dose?

Tadalafil belongs to the phosphodiesterase type 5 (PDE5) inhibitor family, the same group as sildenafil (Viagra). It can be prescribed in two ways: “as needed,” where a 10 mg or 20 mg tablet is taken before expected sexual activity, or once daily, where a low dose is taken every day regardless of when you have sex. MedlinePlus describes these as the two ways of taking tadalafil for ED.

The once-daily strengths are 2.5 mg and 5 mg. The FDA label lists three once-daily uses: ED, the signs and symptoms of BPH, and ED together with BPH. Tadalafil is also sold as Adcirca for pulmonary arterial hypertension, at different doses; MedlinePlus warns that you should use only one tadalafil product at a time.

If you are still deciding between brands, our comparison of Viagra vs Cialis explains how the two drugs differ in onset, duration and food effects.

How daily tadalafil works in the body

During arousal, nerves in the penis release nitric oxide, which raises a messenger molecule called cGMP. cGMP relaxes the smooth muscle of the erectile tissue so blood can flow in. PDE5 breaks cGMP down, and tadalafil blocks PDE5, so erections are easier to get and keep. Tadalafil does not cause an erection on its own; sexual stimulation is still needed.

What makes a daily regimen possible is tadalafil’s long half-life. The FDA label gives a mean terminal half-life of 17.5 hours in healthy men, and with tadalafil daily dosing, steady-state blood levels are reached within 5 days, at about 1.6 times the exposure of a single dose. In practice, this means there is always some active drug in your system once you have been taking it for about a week.

The same enzyme is found in the bladder, prostate and urethra. The EAU explains that raising cGMP there lowers smooth muscle tone, which is why a tadalafil daily dose can also ease urinary symptoms, although the exact mechanism is still not fully understood.

The right tadalafil daily dose depends on why it is prescribed and on your kidney and liver function. The table below summarizes the FDA label.

Situation Once-daily dose (FDA label)
ED only Start 2.5 mg; may increase to 5 mg based on effect and tolerability
BPH only 5 mg
ED and BPH 5 mg
Starting with finasteride for BPH 5 mg for up to 26 weeks
Kidney function CrCl 30–50 mL/min (BPH, ED/BPH) Start 2.5 mg; consider 5 mg
CrCl under 30 mL/min or on dialysis Once-daily use not recommended
Severe liver disease (Child-Pugh C) Not recommended; caution in mild to moderate disease
Taking a strong CYP3A4 inhibitor (e.g. ketoconazole, ritonavir) Maximum 2.5 mg a day

Does 2.5 mg or 5 mg work better for ED?

In the 24-week US trial in the FDA label (287 men), the erectile function score improved by 6.1 points on 2.5 mg and 7.0 points on 5 mg, versus 1.2 points on placebo. The share of attempts in which men kept an erection long enough for successful intercourse rose by 31% on 2.5 mg and 35% on 5 mg, compared with 10% on placebo. In a 12-week trial outside the US, 5 mg improved the score by 9.7 points versus 0.9 on placebo.

In men with diabetes (298 men), both doses also worked, with improvements of 4.8 points on 2.5 mg and 4.5 points on 5 mg versus 1.3 on placebo. The EAU notes that the recommended daily dose is 5 mg and that the evidence it cites for tadalafil daily use is mainly at that strength.

Daily vs on-demand tadalafil

On-demand tadalafil is taken before sex, and the FDA label says the ability to have intercourse is improved for up to 36 hours in most men. A tadalafil daily dose removes the need to time a tablet at all. According to the EAU, there is no clinically significant difference in effectiveness between the two approaches; daily dosing is an alternative for couples who prefer spontaneous sex or expect frequent sexual activity. The choice rests on the patient’s preference and the doctor’s judgment, and the EAU advises reassessing daily treatment periodically.

A 2019 meta-analysis in Sexual Medicine pooled four randomized trials with 1,035 men followed for at least 24 weeks. Taking tadalafil daily gave a slightly greater improvement in erectile function score than on-demand use (mean difference 1.24 points) and lower odds of any treatment-emergent side effect, but there was no significant difference in stopping the drug because of side effects, or in headache, back pain, muscle aches or indigestion. The authors noted that the trials differed considerably in dose and ED severity, so the finding should be read with caution.

Feature Daily (2.5–5 mg) On-demand (10–20 mg)
Timing Same time every day, regardless of sex Before expected sexual activity
Spontaneity High: no planning needed Effect window of up to 36 hours
Treats BPH symptoms Yes (5 mg is FDA-approved) Not an approved BPH regimen
Drug in your system Continuously Only after each dose
Overall ED effectiveness No clinically significant difference (EAU)

The EAU also notes that taking tadalafil daily may improve erections in men who respond only partially to on-demand PDE5 inhibitors. For men who still do not respond, combining daily tadalafil with an on-demand short-acting PDE5 inhibitor is listed as one specialist strategy, but data are limited and it should only be done under medical supervision.

Daily tadalafil for an enlarged prostate (BPH)

BPH is a non-cancerous enlargement of the prostate that can squeeze the urethra. NIDDK lists typical lower urinary tract symptoms (LUTS): trouble starting or emptying the bladder, a weak or interrupted stream, dribbling after urinating, waking at night to urinate, urgency and frequency. NIDDK includes PDE5 inhibitors among the medicines that can relieve some of these symptoms.

Source: NIDDK — Enlarged Prostate (Benign Prostatic Hyperplasia)

How much does it help?

Symptoms are measured with the International Prostate Symptom Score (IPSS), where lower is better. In the FDA label’s 12-week BPH trials, tadalafil 5 mg lowered the IPSS by 4.8 points versus 2.2 on placebo in one study, and by 5.6 versus 3.6 points in another, with improvement visible from the first check at 4 weeks. In men with both ED and BPH, IPSS fell by 6.1 points versus 3.8, and erectile function improved at the same time; in that study the 2.5 mg dose did not significantly improve urinary symptoms.

The EAU calls the benefit modest: pooled data show an IPSS about 1.9 points lower than placebo, and urine flow rate did not differ from placebo in most trials. Even so, the EAU gives a strong recommendation to use PDE5 inhibitors in men with moderate-to-severe LUTS, with or without ED, and notes that tadalafil 5 mg daily is the only drug in this class licensed for male LUTS. Men under 75 appear to benefit more than older men.

Combining with other prostate medicines

With finasteride, the FDA label recommends 5 mg daily for up to 26 weeks; the extra benefit narrowed from 1.7 IPSS points at 4 weeks to 1.0 point at 26 weeks, and benefit beyond 26 weeks is unknown. For alpha-blockers such as tamsulosin, the US label does not recommend the combination for BPH and advises stopping the alpha-blocker at least one day before starting tadalafil daily. The EAU takes a more permissive view, giving a weak recommendation for the combination in men who also want better erections, with no serious adverse events reported. Your urologist will decide which approach fits you.

Side effects of a tadalafil daily dose

The low daily strengths cause fewer and milder side effects than the 10–20 mg on-demand tablets. In the label’s 12-week once-daily ED trials, the most common reactions on 5 mg were headache (6% vs 5% on placebo), indigestion (5% vs 2%), back pain (3% vs 1%), muscle aches (2% vs 1%), stuffy nose (2% vs 0%), flushing (3% vs 1%) and pain in the arms or legs (2% vs 0%). Rates on 2.5 mg were similar or lower.

In the BPH and ED/BPH trials, 5 mg caused headache in 4.1% (vs 2.3%), indigestion in 2.4% (vs 0.2%) and back pain in 2.4% (vs 1.4%). About 4.1% of men taking tadalafil daily for ED stopped because of side effects (vs 2.8% on placebo), and 3.6% in the BPH trials (vs 1.6%).

Rare but serious effects

MedlinePlus lists sudden loss or decrease of vision, sudden loss of hearing (sometimes with ringing or dizziness) and an erection lasting more than 4 hours as serious effects that need urgent care. The NHS adds chest pain and difficulty breathing. For the full picture across all doses, read our guide to Cialis side effects.

Interactions and who should not take it

Combining any PDE5 inhibitor with an organic nitrate is an absolute contraindication, according to the EAU. That includes nitroglycerin, isosorbide and recreational amyl nitrite (“poppers”); nicorandil is also contraindicated. Riociguat, a medicine for pulmonary hypertension, must not be combined either. The combination can cause a dangerous fall in blood pressure.

Taking tadalafil daily matters here, because the drug is always present. The FDA label says that if a nitrate is medically necessary in a life-threatening emergency, at least 48 hours should pass after the last tadalafil dose, with close monitoring. Cleveland Clinic advises going to the emergency room for chest pain within 24 hours of taking a PDE5 inhibitor if you use under-the-tongue nitroglycerin, and telling the staff about the tablet.

Other interactions and cautions

  • Strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir): maximum 2.5 mg daily. The label lists grapefruit juice among substances likely to raise tadalafil levels.
  • Alcohol: five or more units can increase dizziness, fast heart rate, headache and blood pressure drops on standing.
  • Alpha-blockers and blood pressure medicines: can add to blood-pressure-lowering effects; men taking an alpha-blocker for ED should be stable on it before starting at the lowest dose.

The NHS says tadalafil may not be suitable if you have recently had a heart attack or stroke, have low or uncontrolled high blood pressure, liver or kidney problems, sickle cell anemia, a past loss of vision from reduced blood flow to the optic nerve, or a bent penis that makes erections painful. The EAU also lists unstable angina and uncontrolled arrhythmia as reasons not to use PDE5 inhibitors.

Who is a good candidate for daily tadalafil?

Based on the FDA label and EAU guidance, a tadalafil daily dose tends to suit men who:

  • have both ED and bothersome urinary symptoms from BPH, since one 5 mg tablet treats both;
  • prefer spontaneous sex or expect frequent sexual activity, and dislike planning around a pill;
  • respond only partially to on-demand PDE5 inhibitors;
  • have bothersome side effects with higher on-demand doses and want to try a lower continuous dose.

It is less suitable for men who rarely have sex, take nitrates or riociguat, have severe kidney or liver disease, or have unstable heart disease. A proper assessment comes first; our erectile dysfunction guide explains the tests involved.

How to take daily tadalafil correctly

  • Take one tablet at about the same time every day, with or without food, swallowed whole with water.
  • Allow time: steady blood levels take about 5 days to build up.
  • If you miss a dose, MedlinePlus advises taking it when you remember unless the next dose is nearly due; never take two doses or more than one dose a day.
  • Keep a list of your medicines and tell every doctor, dentist and emergency team that you take tadalafil daily.
  • Buy only from a licensed pharmacy with a prescription.

If tadalafil daily tablets do not help enough, other options exist, from switching agents to injections or shockwave therapy; our erectile dysfunction treatment page outlines each step.

When to see a doctor

Get emergency help if you have:

  • an erection lasting more than 4 hours, painful or not;
  • sudden loss or dimming of vision in one or both eyes;
  • sudden hearing loss, ringing in the ears or dizziness;
  • chest pain, fainting or shortness of breath during or after sex (tell staff you take tadalafil, so you are not given nitrates);
  • inability to urinate at all, blood in the urine, or painful urination with fever and chills.

Book a routine review if side effects persist, if the effect fades, or before adding any new medicine.

FAQs about tadalafil daily dose

Is 5 mg of tadalafil daily enough for ED?

For many men, yes. In the FDA label’s 24-week trial, 5 mg daily raised the erectile function score by 7.0 points versus 1.2 on placebo, and the EAU names 5 mg as the recommended daily dose. The label starts ED at 2.5 mg and allows 5 mg depending on response. If 5 mg is not enough, your doctor may consider other strategies.

How long does daily tadalafil take to work?

Tadalafil reaches peak blood levels a median of 2 hours after a dose, and with tadalafil daily use, levels build up to a steady state within about 5 days. For urinary symptoms of BPH, the label’s trials showed improvement at the first scheduled check, at 2 to 4 weeks.

Can I take daily tadalafil at night?

Yes, morning or evening both work; what matters is taking tadalafil daily at about the same time. The FDA label says it can be taken with or without food. Some men choose a time linked to a daily habit, such as brushing their teeth, so they do not forget. Never take an extra dose to make up for a missed one.

Sources: DailyMed (FDA label) — CIALIS prescribing information; MedlinePlus — Tadalafil

Is it safe to take tadalafil every day long term?

Taking tadalafil daily is an approved long-term regimen, and the EAU found no randomized or open-label studies showing more heart attacks with PDE5 inhibitors. A 2019 meta-analysis of trials lasting at least 24 weeks found daily use was not less tolerable than on-demand use. The EAU still recommends reassessing daily treatment periodically with your doctor.

Sources: EAU Guidelines — Management of Erectile Dysfunction; Sexual Medicine (PMC) — Zhou et al., 2019

Does daily tadalafil shrink the prostate?

There is no good evidence that it does. The EAU states there is limited information on any reduction in prostate size and no data on disease progression. Tadalafil eases symptoms by relaxing smooth muscle in the bladder, prostate and urethra, whereas finasteride and dutasteride act on prostate growth. That is why they are sometimes combined.

Source: EAU Guidelines — Management of Non-neurogenic Male LUTS: Disease Management

Can I drink alcohol on a tadalafil daily dose?

The FDA label warns that substantial alcohol, five units or more, raises the risk of dizziness, a faster heart rate, headache and a drop in blood pressure on standing. Because taking tadalafil daily keeps the drug in your blood continuously, heavy drinking deserves extra caution.

Source: DailyMed (FDA label) — CIALIS prescribing information

Can I take Viagra on top of daily Cialis?

Not on your own. Both are PDE5 inhibitors; the FDA label says combinations with other PDE5 inhibitors have not been studied and tells patients not to take Cialis with them. The EAU mentions tadalafil daily plus an on-demand short-acting PDE5 inhibitor only as a specialist strategy for men who do not respond to standard treatment, with limited supporting data. Discuss it with a urologist first; see our guide to sildenafil side effects for what to watch for.
